Ok I got it figured out. Here are the pictures for installing it and the model number for the U.S.

To use the antenna feature you have to take it out of your case so the back of the phone makes contact with the cradle. You can leave it in the case (I have a thin Incipio case) and still connect it, have it charge, access apps, etc.
