I am setup with my key as the primary user.

When I get in the car everything changes to my profile no issues.

When my wife gets in the car with her key it does not automatically select her profile even though her key is locked to her profile and comes up as a guest user so she has to hit her profile for things to change into her settings

Am I missing a setting somewhere ?

Is your wife registered as a primary user and a guest profile under your primary user? I think that might be what’s causing the confusion. You’ll need to Link the key to either one of the other, not both.

Can I ask, what are you using for your key? Phone, Watch, FOB, Key card, etc. Each of these various “keys” battling for priority also causes confusion in my experience!
